Mountainview Family Practice

Mon-Fri  8 a.m. – 5 p.m.
Evening and weekend visits available by appointment

Normal appointment hours are 8:00 a.m to 5:00 p.m., Monday through Friday. Visits are by appointment. Saturday hours are available by appointment and staffed by the provider on call. Emergencies are accepted and less urgent problems will be worked in to our scheduled appointments as soon as possible. If you are unable to keep your appointment, we will be happy to reschedule you. We do ask that you notify us as early as possible (24 hours notice preferred) so we can offer your time to another patient.

New patients should arrive 30 minutes prior to their appointment time to fill out patient information and medical data forms. Please bring your insurance cards with you. We will be making a copy for your file.  Immunization records for children should also be copied for the file.

There will be times when emergencies at the hospital will interrupt the doctors office hours. This can result in delays or possible rescheduling of your visit. We will try to contact you as early as possible to notify you of any changes in your appointment time should they occur.

Our phones are answered by office personnel from 8:00 a.m. to 5:00 p.m., Monday through Friday. The clinical staff is available to triage your call from 8:00 a.m. to 5:00 p.m. If the clinical staff is unavailable to take your call you may leave a detailed and confidential message with the receptionist or ask to be transferred to their voice mail to leave a detailed or confidential message. Your calls will be returned promptly as voice mail is checked every hour. In the case of emergency the clinical staff and physician are available to take your call. Please inform the receptionist you are calling with an emergency.

We suggest calling our office after hours prior to going to the emergency room. You will connected directly to our phone service. Many calls can be handled by the provider on call, thus saving you the emergency room charges. Many insurance’s also require that you be directed to the emergency room by your physician before payment of those charges will be considered.

As family physicians our doctors strongly believe in the importance of family life. There will be times when another doctor will be covering calls for you doctor to allow them time with his or her family.

Requests for prescription refills should be called directly to your pharmacy. They will contact us for approval. Please allow 48 hours for processing. Refills on the weekend will be limited to an amount to cover you until the office reopens on Monday. Narcotics or pain medications will not be refilled on the weekend.

Your charges are determined by the time spent and vary with the severity and complexity of the problem. We request payment at the time of your visit. Payment arrangements must be made in advance of services.  As a courtesy to you we will bill most insurances. You are responsible for the co-pays set by your health insurance and they are payable at the time of service. All other charges not covered by your insurance are payable upon receipt of our monthly statement. To ensure prompt and correct billing please notify the staff of any changes in your health care coverage. We are participating providers on most managed care and insurance programs. If you have questions about your coverage, please call our well-informed staff and we will try to answer your questions.

Occasionally we may refer you for consultation with another physician or specialist to obtain additional insight to your health care. Should this become necessary the referring provider will receive information from your file. Together, your provider and the consultant will develop a care plan.

If you desire a second opinion, please schedule an appointment and talk to your provider. We will be able to suggest several doctors and provide them with your medical records as needed.

The consultant will bill you directly for their services. We will initiate the Managed Care referral process as needed. Please confirm with your insurance prior to the visit that all referrals are in place.

Your medical record is strictly private. Information will not be released without your written permission. All records are released in accordance with the laws and regulations regarding medical information.
